Vaccinations for 12-15 years of age

What identification do we need to get a vaccination?

According to one of our partners at LA County Public Health, "parents should bring whatever they have that can help them identify their child. If the child has an old school ID that will work fine. The child will also need something to verify their age. Passports, birth certificates, adoption certificates, etc. should be perfect for this. Anything with the students name and date of birth.

If the parent can bring identification for themselves that is also helpful (they do not need to be US citizens or in the US legally to get vaccinated). If the student does not have photo ID they will still be allowed to get the vaccine. However they will need to prove identity in some way."

Please note that this is for informational purposes for those students and parents that would like to be vaccinated. There is currently not a mandate for MUSD students to be vaccinated for COVID.
